New Student Registration Information
Registration Information

Registration is held every day from 8:00 am - 4:00 pm at the Pawtucket Administration Building, 286 Main St., Pawtucket, RI. 



The following documents are required to register your child for school:

                          

1.       Proof of Residency: Bank statement, health insurance statement, payroll stub, federal, state or local government correspondence, car insurance bill, utility bill, lease (choose three). All proof of residency documents must be current (within 30 days) and have your name and current address on it.

2.       Original Birth Certificate or Passport (for proof of age)

3.       Parent Picture ID (license or passport not expired)

4.       Health information requirements:

Up to date immunization records along with the current physical, vision screening and lead screening documentation.

Los siguientes documentos son requisitos para matricular a los niños en la escuela :

 

  1. Prueba de domicilio:  Una declaración del estado de cuenta bancaria, una declaración del estado del seguro de salud,  un comprobante del cheque del trabajo, una correspondencia gubernamental, ya sea federal, estatal, o de la ciudad, factura del seguro de carro, factura de utilidades, (la luz, gas, cable o teléfono),  copia del arrendamiento, (escoge tres).    Todos  los documentos de  pruebas de domicilio tienen que estar corriente (de los últimos 30 días) y tienen que tener su nombre con la dirección corriente.  
  2. El Certificado de nacimiento original (para verificar la edad) 
  3. Identificación con foto del padre  (licencia o passaporte que no este vencido)
  4. INFORMACIÓN DE REQUISITOS DE SALUD:

              Registros de vacunas actualizados junto con un examen físico, visual actual, y el documento del 

              examen de plomo.

Os documentos mencionados abaixo são requeridos para registrar seu filha/filho para escola:

 

  1.   2. Certificado de Nascimento Original ou Passaporte (para prova de idade) 
  3. Identificação com foto do Pai/Guardiao (carteira ou passaporte sem ter expirado)
  4. REQUERIMENTOS DE INFORMAÇÕES DE SAÚDE:

              Records de vacinas atuais junto com o físico atual, testes de visão e documentação de teste de chumbo

Registration Health Requirements
Reminder  for Parents of Students entering Kindergarten, Grade 7 and 12.

In accordance with the Rhode Island Department of Health Rules and Regulations Pertaining to Immunization and Testing for Communicable Diseases (R23-1-1MM),all children entering school are required to have the following immunizations:

Required immunizations for students

Requirements for children entering pre-kindergarten, child care, or day care centers
These requirements only apply to children in facilities licensed by the Rhode Island Department of Children, Youth, and Families.
  • 4 doses of DTaP (diphtheria, tetanus, pertussis) vaccine
  • 1 dose of flu vaccine each year
  • 3 doses of hepatitis B vaccine
  • 3 doses of Hib (Haemophilus influenzae type b) vaccine
  • 1 dose of MMR (measles, mumps, rubella) vaccine
  • 4 doses of pneumococcal conjugate vaccine (not routinely given to healthy children 5 years of age and older)
  • 3 doses of polio vaccine
  • 1 dose of varicella (chickenpox) vaccine

Requirements for children entering kindergarten

A child who is entering kindergarten must have met the pre-kindergarten immunization requirements, plus:
  • 1 dose of DTaP (diphtheria, tetanus, pertussis) vaccine
  • 1 dose of MMR (measles, mumps, rubella) vaccine
  • 1 dose of polio vaccine
  • 1 dose of varicella (chickenpox) vaccine

Requirements for children entering 7th grade

A child who is entering 7th grade must have met the pre-kindergarten and kindergarten immunization requirements, plus:
  • 1 dose of HPV (human papillomavirus) vaccine
  • 1 dose of meningococcal conjugate (meningitis) vaccine
  • 1 dose of Tdap (tetanus, diphtheria, pertussis) vaccine

Requirement for children entering 12th grade

A child who is entering 12th grade must have met the pre-kindergarten, kindergarten, and 7th grade immunization requirements, plus: 

  • 1 dose of meningococcal conjugate (meningitis) vaccine - booster dose

If you do not have health insurance or a regular doctor, your child can get immunized at the St. Joseph Center for Health and Human Services
(21 Peace Street, Providence, Rhode Island; 401-456-4359).


Walk-in's are welcome at St. Joseph on the following dates and at the following times:
  • Mondays: 10 AM - 3 PM
  • Tuesdays: 12 PM - 4 PM
  • Wednesdays: 12 PM - 4 PM
  • Thursdays: 8 AM - 4 PM

Immunizations at other times can be arranged by appointment. Please the child's immunization history to his or her visit.

  •   Students do not need to get the immunizations listed above if they have already received them. This can be demonstrated with an immunization record, a medical passport, or any other official record that shows the month, day, and year of the vaccinations.

  • Students do not need to get the immunizations listed above if they cannot receive them for medical reasons. A healthcare provider can provide paperwork stating that someone cannot be vaccinated against a particular disease for medical reasons.
